Try moving the bars?
Now I've also had back and shoulder issues on the bike because I have arthritis in every joint and spondylitis which is like arthritis for stuff in the spine that's not a joint. So having put the foot pegs back where they were I then experimented with other bars. I tried lower bars which did not work at all well. I dislike high bars but cannot lean forwards and support weight on my arms for long. So I again reverted to the stock bars. I moved them back as far as they would go and as far forwards as the controls would permit (they're pegged), until I found a comfortable position. Perhaps as little as one inch further forwards than they were originally fitted, with the grips angled just a little downwards from parallel to the ground. I do have to remind myself to sit up straight, slight forwards lean, not slumped like a sack of potatoes. I also tend to ride with the ball of the foot on the peg not the instep, to let me put more weight down there.
