Aha, now it is clearer - You have too much slack so the ferrule drops out. So the outer is too short, or the inner too long . Perhaps you are missing a slotted ferrule from the other end of the cable? (this will make the outer longer and the inner shorter, - so less free play)

Spence just a chance the problem will disappear if you reroute the cable. Don't have it going through the cassette, any tight bend effects the return action and shutting the throttle seems to force the cable out of the ferrule..

I ran mine down the headstock with a gentle bend under the tank, not down the tank tunnel..

I usually put a small cabletie around the brass ferule so that if the cable sticks you don't loose the ferule when it drops out
