They should be in tight but as you say you have exhausted all other ideas and I have heard of a loose one some time back and with Indian machining anything is possible. There is a tappet guide removal tool because they are fitted fairly tight, but to use this the barrel needs to be removed. In your case if the guide is the problem I would get the motor hot and remove the timing cover and with a screwdriver see if you can get any movement upwards on the guides. You probably wouldn't get any with a cold enging and if you did the guides would certainly be floating when the motor was warm. No guarantee this is the problem but is somewhere to start
